Member-Get-a-Member and Ambassadors
If you enrolled a nurse into Third District, and want to enter our drawing to win $300, you must make a copy of the front-page application of the nurse you enrolled and put your name, phone and email address at the top. Indicate that you want to enter the Member-Get-a-Member drawing. Mail copy of application to Membership Director, Third District Nurses, 4445 West 77^th St. Suite 121, Edina MN 55435. You may also fax it to us at (952) 920-8689. Thank you for supporting Third District and Good Luck! We hope you win the $300.

If you are already a member of MNA and just want to add Third District to your membership, please take the following steps.

1. Contact MNA (651) 646-4807 or Third District (952) 920-9860 for a membership form.

2. Complete the front page with your name, social security number, address, email, RN license and facility name. You do NOT need to complete all the demographic information unless it has changed.

If you are including updated information, please indicate on the side of the form. At the bottom of the front-page check the “District Membership Option” box that says “MNA and district membership”. WRITE IN “3^rd” just above the word district. This indicates your intention to join.

3. On the second page your must check the top box following “If you are an RN:” It indicates that you want “MNA and Third District” dues to be deducted. You must put in facility name, print your name and sign.
